Athletic Participation and Physical Form - MUST BE completed and turned into the Athletic Director's office for any athletic participant at Mill Creek High School.  This form includes a current physical, insurance coverage information and parental consent agreed to and signed. ALL 4 PAGES OF THIS MILL CREEK DOCUMENT MUST BE COMPLETED. Please download and complete the Concussion Management Form also and turn in with physical form.
